Arjan Diepstra
dr.

As a hematopathologist, I work on diagnostics of all types of hematological malignancies using a comprehensive panel of different techniques. Moreover, my main research interest involves Hodgkin lymphoma, with a strong focus on interactions between tumor cells and the microenvironment. In addition, I also have a long standing interest in genetic susceptibility. My areas of expertise are: immunology, tumor cell biology, genetic association studies and molecular diagnostics in pathology. I actively participate in international (EORTC) and national (HOVON) clinical trials.

R-miniCHOP versus R-CHOP in elderly patients with diffuse large B-cell lymphoma: A propensity matched population-based study
Published in: American Journal of Hematology
For elderly frail patients with diffuse large B-cell lymphoma (DLBCL), an attenuated chemo-immunotherapy strategy of rituximab, cyclophosphamide, doxorubicin, vincristine and prednisolone (R-miniCHOP) was introduced as a treatment option as from 2014 onward in the Netherlands. Although R-miniCHOP is more tolerable, reduction of chemotherapy could negatively affect survival compared to R-CHOP. The aim of this analysis was to assess survival of patients treated with R-miniCHOP compared to R-CHOP. DLBCL patients ≥65 years, newly diagnosed in 2014-2020, who received ≥1 cycle of R-miniCHOP or R-CHOP were identified in the Netherlands Cancer Registry,...

Health-related quality of life is linked to the gut microbiome in kidney transplant recipients
Published in: Nature Communications
Kidney transplant recipients (KTR) have impaired health-related quality of life (HRQoL) and suffer from intestinal dysbiosis. Increasing evidence shows that gut health and HRQoL are tightly related in the general population. Here, we investigate the association between the gut microbiome and HRQoL in KTR, using metagenomic sequencing data from fecal samples collected from 507 KTR. Multiple bacterial species are associated with lower HRQoL, many of which have previously been associated with adverse health conditions. Gut microbiome distance to the general population is highest among KTR with an impaired...
Elevated serum TARC levels precede classic Hodgkin lymphoma diagnosis by several years
Published in: Blood
Tumor cells in classic Hodgkin lymphoma produce high quantities of the chemokine TARC. We measured TARC levels in pre-diagnostic serum samples and found strikingly increased values in the vast majority of patients, up to over six years before diagnosis.

Real-world routine diagnostic molecular analysis for TP53 mutational status is recommended over p53 immunohistochemistry in B-cell lymphomas
Published in: Virchows Archiv : an International Journal of Pathology
Previous studies in patients with mature B-cell lymphomas (MBCL) have shown that pathogenic TP53 aberrations are associated with inferior chemotherapeutic efficacy and survival outcomes. In solid malignancies, p53 immunohistochemistry is commonly used as a surrogate marker to assess TP53 mutations, but this correlation is not yet well-established in lymphomas. This study evaluated the accuracy of p53 immunohistochemistry as a surrogate marker for TP53 mutational analysis in a large real-world patient cohort of 354 MBCL patients within routine diagnostic practice. For each case, p53 IHC was assigned to one...
